Acceptance of Terms ## By visiting our website or scheduling care with us, you agree to these Terms. If you do not agree, please do not use this website or our services. We update these Terms occasionally — the current version is always posted here. Last Updated · January 1, 2025 ### Medical Disclaimer IV vitamin therapy provided at RevIVe Wellness & Vitamin Infusions is administered by a licensed nurse practitioner and supported by pharmaceutical-grade nutrients sourced from FDA-regulated 503(a) and 503(b) compounding pharmacies. Our services are intended to support hydration, nutrition and overall wellness. They are **not** a substitute for diagnosis or treatment from your primary care provider, oncologist or specialist. Statements on this website have not been evaluated by the FDA and are not intended to diagnose, treat, cure or prevent any disease. ### Appointments, Cancellations & Late Arrivals - We respectfully request **24 hours notice** for cancellations or reschedules. - Late cancellations or no-shows may be subject to a $50 fee. - Custom infusions and NAD+ require additional advance notice (please call several days ahead). - Arrivals more than 15 minutes late may need to be rescheduled to honor other guests' time. ### Payment, HSA / FSA & Refunds Payment is collected at the time of service. We accept major credit cards and HSA/FSA cards where eligible — please confirm with your plan administrator. Because IV therapy is administered in real time using individually compounded ingredients, infusions and injections are **non-refundable once initiated**. If you become unwell or change your mind before your IV begins, we'll do our best to accommodate a rescheduled visit. ### Eligibility, Intake & Informed Consent Every guest must complete an intake form and provide informed consent prior to any infusion or injection. We reserve the right to decline or modify a treatment if it is determined to be unsafe or inappropriate based on your medical history, vital signs or current condition. Common reasons for declining include uncontrolled cardiac, kidney or liver disease, certain electrolyte conditions and pregnancy where indicated. ### Gift Certificates - Gift certificates are non-refundable but fully transferable. - Certificates do not expire and may be applied to any service of equal value. ### Intellectual Property All content on revivecaz.com — including the RevIVe name and logo, photography, copy, infusion menu descriptions and design elements — is the property of RevIVe Wellness & Vitamin Infusions and may not be reproduced, distributed or used without written permission. ### Use of This Website You agree to use this website lawfully and respectfully. You may not attempt to gain unauthorized access, interfere with site operation, scrape content for commercial use or post unlawful, harmful or harassing material through any contact form. ### Limitation of Liability To the fullest extent permitted by law, RevIVe Wellness & Vitamin Infusions, its owners, employees and contractors shall not be liable for any indirect, incidental, consequential or punitive damages arising out of your use of this website or any information presented on it. Clinical liability is governed separately by your informed consent and applicable New York State law. ### Governing Law These Terms are governed by the laws of the State of New York, without regard to its conflict-of-law principles.
